Common forms

Authorization for Administration of Medication During School Hours

  • This form must be on file with the school for any student to use medication, even over-the-counter medications, while at school. This includes medications, such as inhalers, that the student will carry with them. This policy if for the protection of all students on campus. 
  • If your child has a more complex medical condition, such as diabetes, anaphylaxis, or seizures, please contact the school nurse before getting orders from your doctor as additional information may be needed. The school nurse will help with coordination of health needs between the school and the doctor.

Calaveras County Public Health

Offers clinical services including: immunizations, pregnancy testing, TB skin tests, and HIV testing.

Can also provide resources for additional services and have a variety of educational programs.

Medical Insurance and Financial Assistance Programs
